Comprehensive shipping guidance for Schedule II-V, Legend, and OTC products, presented for your reference.

  • Fill out the “Schedule II Return Request” form.
  • Fax the completed form to: 630-277-8340 or email to custserv@pharmreturns.net.
  • Within a week you will receive a DEA-222 Form with instructions and shipping supplies.
  • Follow the instructions on the Schedule II Program letter.
  • Place II’s in the tamper-evident bag and place inside of any box from your facility.
  • Include a photocopy of the DEA-222 Form inside the box or inside the tamper-evident bag.
  • Fill out your return address on the prepaid FedEx label & call the (800) phone number on the back of the label.
  • Keep the FedEx shipper receipt (FedEx tracking number).

NOTE: You cannot send any Schedule II product until you have received your DEA-222 Form.
NOTE: Schedule II product MUST be shipped separately from all other items.

  • Fill out the “Schedule III-V Drug Report” form.
  • Place all III-V’s in the tamper-evident bag and place inside of any box from your facility.
  • Include a copy of the “Schedule III-V Drug Report” form inside the box or inside of a packing list enclosed envelope.
  • Fill out your return address on the prepaid FedEx label & call the (800) phone number on the back of the label.
  • Keep the FedEx shipper receipt (FedEx tracking number).

  • Pull your outdates 3 months ahead.
  • Fill out the “Legend Drug Report” form.
    Place all Legend/OTC/Hazmat medications inside of any box from your facility.
  • Include a copy of the “Legend Drug Report” form inside of the box or inside of a packing list enclosed envelope.
  • Fill out your return address on the prepaid FedEx label & call the (800) phone number on the back of the label.
  • Keep the FedEx shipper receipt (FedEx tracking number).

On the back of the prepaid FedEx label is a (800) phone number, call to schedule pick-up using the prompts.

Yes. However, please ensure the CIII-V’s are consolidated within the tamper-evident bag and place the bag inside the box containing your Legend/OTC product. It is also important to make sure you include copies of the “Legend Drug Report” and “Schedule III-V Drug Report” forms within the shipment.

You will receive the DEA-222 form with instructions and materials within 7 business days of submitting the request. Our team will also send an email with the FedEx tracking number for the DEA-222 form for your reference.

You can re-submit the “Schedule II Return Request” form and we will re-issue a new DEA-222 form for your facilities schedule II medication. Please reach out to our customer service team if you have any issues with outdated DEA-222 forms.
